Under a minute
Six questions at most, all yes or no. We do not ask for your name, your email or your phone: the result is worked out in your browser and never reaches us.
First, for safety
Have you noticed any of these in the last few days?
- Sudden loss of vision in one eye, even if it passed quickly
- A shadow or curtain covering part of what you see
- A sudden shower of floaters, or flashes of light
- Straight lines looking bent from one day to the next
- Severe pain in one eye, with halos around lights or nausea
This should not wait for an appointment
What you have marked may need attention today, and it is not something an optician can resolve. Go to an eye casualty department or call 112.
If you marked it by mistake, you can go back and carry on.
How old are you?
It only decides which questions you get. If you are answering for a child, choose the child’s age.

Why we say this
What we would do at the visit
- Measure how you see far and near, with and without your current glasses.
- Check the health of the eye, including intraocular pressure.
- Compare it with what we recorded at earlier visits. And if anything falls outside what we would expect, a report for the ophthalmologist.
This is a screening questionnaire, not a diagnosis and not an eye examination. It is here to tell you whether a visit is worth your time, and it does not replace seeing an optometrist or an ophthalmologist.
